Orthopaedic Physician Assistant will provide a full range of diagnostic and therapeutic procedures for patients in the outpatient, inpatient, and operating room settings, provide comprehensive orthopaedic medical care to include non-operative and pre-operative patient evaluation and care as well as post-operative care in the Orthopaedic Clinic, Emergency Department, and inpatient floors at David Grant Medical Center (DGMC), Travis AFB, California. The PA-C will work under supervision and guidance of the supervising physician or alternate.
Duties include (but are not limited to): independently obtaining medical histories, making diagnosis and treating patients, making arrangements for further care if appropriate, ordering and interpreting lab and diagnostic tests, prescribing medication, managing orthopaedic inpatients, providing on-call consultation services for Emergency Department and inpatient floor patients, staffing fracture clinic, and assisting the orthopedic surgeon in the operating room. Responsible for approving Durable Medical Equipment/Prosthesis consults in the VA medical record system for items used in surgical procedures and post-operative rehabilitation. Provides primary or specialty medical care with the cognizance or supervision of a physician. Provides services by collecting data required for diagnosis and performing procedures and services. Obtains and evaluates medical histories, performs physical examinations, performs therapeutic and diagnostic procedures, and orders laboratory studies, radiographs, electrocardiograms, and other special examinations. Collects specimens for pathologic examination. Analyzes and interprets data, formulates problems lists, and establishes plans for solution of clinical problems. Participates as educational staff for orthopaedic physician assistant training program.
Procedures include: Intra-articular and extra-articular injections, closed reduction of fractures, casting and splinting. He/she will be expected to admit and discharge patients from the hospital on behalf of staff surgeons, coordinates patient care with other members of health care team. First assists on surgical cases, closes surgical and traumatic wounds, initiates consultation requests to other health care professionals, accomplishes narrative summaries and other required patient documentation. Instructs other members of the health care team, personnel assigned to health care team on preventative medicine programs and other appropriate medical activities and functions, or as directed by a physician. The PA-C will provide education, counseling and emotional support for patients and families receiving continuing care.
Prescriptive privileges will follow VA, DoD, and medical center guidelines. Prescribing of drugs for inpatients is restricted to routine medications and DEA class II, III, and IV controlled substances. He/she will be authorized to prescribe all routine medications as listed in the VA and DoD Pharmacy Formulary with the exception of chemotherapeutic agents, and special medications restricted to approval by another section or department.

Physical Requirements: Use and function of all normal motor and sensory anatomy for provision of patient care, reading, writing, listening, observing/seeing, heavy lifting, bending, pulling, pushing, stooping, squatting, walking, standing, use of fine motor muscles; visual and auditory acuity; emotional intelligence to respond to stressful and/or emergent situations and crises appropriately and calmly; self-care skills to maintain physical, psychological, and emotional health. Examples include: heavy lifting of 45 pounds and over, heavy carrying of 45 pounds and over, straight pulling, pushing, reaching above shoulder, use of fingers, both hands required, walking, standing, kneeling, repeated bending, ability for rapid mental and muscular coordination simultaneously, near vision correctable at 13" to 16" to Jaeger 1-4, far vision correctable in one eye to 20/20 and to 20/40 in the other. Must be able to stand for long periods in the OR. Up to 4 cases a day
Drug Testing: This is a testing designated position. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment. Applicants who refuse to be tested will be denied employment with the VA. Applicants who test positive for illegal use of drugs will not be appointed.

BASIC REQUIREMENTS a. Citizenship. Citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3, g, of this part.) b. Education. The candidate must meet the following: (1) Graduate of a PA training program, or of a surgical assistant program which is accredited by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) or one of its predecessor organizations; and (2) A Bachelor's degree from an accredited PA training program or in another health sciences related field from an accredited college or university recognized by the U.S. Department of Education. c. Certification (1) Current and continuous certification by the National Commission on Certification of Physician Assistants (NCCPA) is required for employment in VHA. (2) Recent PA graduates who are eligible to take the Physician Assistant National Certifying Examination (PANCE) may be employed by VA on a temporary basis (38 U.S.C. 7405), for a period not to exceed two years, as provided in VA Handbook 5005, Part II, Chapter 3, Section G, Paragraph 5. d. English Language Proficiency PAs appointed to direct patient-care positions must be proficient in spoken and written English as required by 38 U.S.C. 7402(d). GRADE REQUIREMENTS. In addition to the basic requirements stated in paragraph 2 above, the following qualifications criteria must be met in determining the appropriate grade for appointment or promotion eligibility. Requirements for appointment or promotion to a specific grade are based on the candidate's ability to demonstrate the elements described. a. Full Grade. In addition to meeting the basic requirements in paragraph 2, individuals appointed to this grade must meet all the following requirements: (1)Experience / Education. None beyond the basic requirements in paragraph 2 of this standard.
b. Intermediate Grade. In addition to meeting the basic requirements under paragraph 2 above, candidates must meet all requirements described in this standard for Full Grade through VA employment or non-VA PA positions with equivalent scope and complexity. Individuals appointed or advanced to Intermediate Grade will have demonstrated the ability to analyze and solve more complex medical problems and meet the following requirements: (1)Experience / Education (a)1 year of professional practice as a PA; or (b)Successful completion of a PA post-graduate residency program accredited by ARC-PA or by the U.S. Armed Forces; or (c)A Master's degree from a PA training program certified by ARC-PA or in another health-related field from an accredited college or university recognized by the U.S. Department of Education.
c. Senior Grade. In addition to meeting the basic requirements under paragraph 2 above, candidates must meet all requirements described in this standard for Intermediate Grade through VA employment or non-VA PA positions with equivalent scope and complexity. Individuals appointed or advanced to Senior Grade will have demonstrated the behaviors or responsibilities listed in one or more of the following categories: Clinical Proficiency, Education, Professional Development, Management/Administrative, and Research, as applicable to the assignment (1) Experience / Education (a) 3 years of professional practice as a PA; or (b) 2 years of professional practice as a PA and successful completion of a PA post-graduate residency program accredited by ARC-PA or by the U.S. Armed Forces; or (c )2 years of professional practice as a PA and a Master's degree from a PA training program certified by ARC-PA or in another health-related field from an accredited college or university recognized by the U.S. Department of Education; or (d) A Doctoral degree in a health-related field from an accredited college or university recognized by the U. S. Department of Education.
d. Chief Grade (1) General. Chief grade is restricted to selected leadership positions. The individual's qualifications as well as the scope and complexity of the assignment are considered. (2) Education. The preferred minimum education for this grade is a master's degree from a PA training program certified by the ARC-PA or a master's degree in a health care occupation or a health-related science and a baccalaureate degree from a PA training program certified by the ARC-PA. However, experience as a PA may be substituted for the master's degree where the individual has an equivalent knowledge of the profession and has successfully undertaken difficult or complex clinical, administrative, research and/or educational assignments which have required an unusual degree of leadership and competence. (3) Experience. Professional practice characterized by progressively more responsible leadership assignments in clinical, administrative, educational and/or research areas. An individual can demonstrate these abilities after approximately 8 years of practice as a PA. Practice must have consistently demonstrated all of the following behaviors (if applicable to the assignment): (a) Establishes and maintains relationships within the service, with facility management and other services that result in improved patient care. (b) Accepts substantial and continuing responsibility and accountability for planning, organizing, coordinating and controlling a complex integrated health care program. (c) Makes decisions which serve to enhance the distinctive and contributory role of PAs within the local institution and the total health care system. (d) Participates in long range program planning and model development for effective delivery of health services. (e) Facilitates the delivery of patient care in collaboration with health professionals, health managers and other health care personnel. (f) Assures implementation, maintenance and compliance with standards of professional practice of all health care personnel within the program.
Preferred Experience:experience as a practicing Physician Assistant in Orthopedics is preferred but not mandatory.
