Job Title: Licensed Vocational Nurse (LVN)
Under the direction of the Director of Patient Care Services / Director of Nursing, the Licensed Vocational Nurse (LVN) provides services to patients in accordance with the State Nurse Practice Act, agency policies, and accepted professional standards of practice utilizing a comprehensive base of nursing theory and nursing process, and communicates and documents observations and assessments.

Duties and Responsibilities:
Ensures quality and safe delivery of home health services to patients/clients and families/caregivers. Implements plan of care initiated by the case manager. Provides accurate and timely documentation consistent with the plan of care. Assesses and provides patient/client and family/caregiver education and information pertinent to diagnosis and plan of care. Notify the patient's attending physician, dentist or podiatrist and other professional persons and responsible staff of significant changes in the patient's condition. Prepares clinical and progress notes. Assists the physician and registered nurse in performing specialized procedures prepares equipment and materials for treatment observing aseptic technique as required, and assists the patients in learning appropriate self-care techniques. Assumes responsibility and accountability for the practice of professional nursing in accordance with the State Nurse Practice Act, and standards for home health nursing. Consistently demonstrates competency with technical nursing skills according to personal and legal scope of practice. Assessment skills as applied to the patient, family/support system, and environment. Teaching skills according to the patient's needs and within the context of the patient's environment, value system, and physical/emotional status. Medication administrations. Initiates actions to improve the quality of patient are. Takes appropriate actions independently and responsively in home care situations. Initiates referrals as needed to other health team members when delegated by the Case Manager. Promotes personal safety and a safe environment for patients and coworkers. Demonstrates knowledge of safety and infection control practices by compliance with established policies and procedures. Demonstrates safe practice in the use of equipment. Assesses safety of environment and takes initiative to help prevent accidents and promote safety. Participates in ongoing, professional self-development. Participates in all mandatory education per agency guidelines. Identifies needs for personal professional growth. Demonstrates competency in areas of assignment or identifies the need for additional training (pediatrics, infusion, specialized equipment, etc.) Performs other duties as assigned. Qualifications/Requirements:
CONTINUING EDUCATION REQUIREMENTS
Agency personnel are expected to participate in appropriate continuing education as may be requested by their immediate supervisor. In addition, agency personnel are expected to accept personal responsibility for other educational activities to enhance job related skills and abilities. All agency personnel must attend mandatory educational programs.
Minimum of one (1) year nursing experience. LICENSES/CERTIFICATIONS:
Current license as a Licensed Vocational or Practical Nurse in the State of California. Maintains a current CPR certification if required by applicable law, rule, ordinance, or regulation. Current drivers license, good driving record, and reliable transportation. RESPONSIBILITY/SUPERVISORY ACTIONS:

The ability to perform highly detailed work with sustained attention and care while providing/obtaining information on numerous inquiries is paramount to success in this position.
