Kaiser Permanente Enterostomal Case Manager, On-Call in Redwood City, California

Counsels and case manages patients with ostomy-related conditions, including wound management, draining, pressure sores, and incontinence problems. Assists in the provision of current standards of practice for ostomy patients. Does not provide direct patient care on a routine or semi-routine basis.
Authorizes patients currently being cared/followed for ostomy and wound care supplies through the approved KP ostomy benefits program.
Assesses individual patient needs and goals, and provides educational programs to physicians, nursing, and other paramedical professionals.
Evaluates currently used products for ostomy and wound care.
Conducts continuing educational programs for physicians and nursing personnel.
May provide guidance and supervision to Staff RNs providing direct patient care to patients with ostomy-related conditions.
Pre-counsels patients for surgery and follow-up after surgery.
Sees patients for appointments regarding problems concerning ostomy management, wound management and other related concerns, rather than making an appointment with their physician whom it is difficult to maintain total familiarity with new equipment and techniques being used in ostomy care.
Schedules patients in the outpatient department for pressure sore and incontinence management.

Bachelor's degree in nursing or health related field, OR four (4) years of experience in a directly related field.
Graduate of an accredited school of nursing.
License, Certification, Registration
Current RN license.
Certification as an E.T. Nurse from an accredited E.T. Nursing Education Program.
Board Certification in Enterostomal Therapy Nursing.
Significant knowledge of pediatric and adult patient teaching principles.
Extensive knowledge of the gastrointestinal, genitourinary and integumentary systems including pathophysiology.
Excellent communication skills.
Intense desire to teach self-care to patients.
Fine motor dexterity.
Must be able to work in a Labor/Management Partnership environment.
Clinical nursing experience in caring for patients with abdominal somas, draining wounds, fistulas, pressure sores and incontinence.
Master's degree preferred.
