Full-Time Hospice Registered Nurse Clinical Manager/RN

Work Schedule: Monday through Friday

The Hospice Clinical Manager is responsible for ensuring patient care is coordinated and delivered appropriately, and for supervising clinical personnel.

  • Receive case referrals, and review patient information to determine hospice needs.

  • Review and evaluate each case through home visits, conferences, record review, and the service provided by clinicians.

  • Assist clinicians in establishing immediate and long-term therapeutic goals, setting priorities, and developing a plan of care.

  • Instruct and guide clinicians to promote more effective performance and delivery of quality home care services.

  • Attend case conference meetings to facilitate coordination of care and interdisciplinary group involvement.

  • Participate in orientation of new staff, annual evaluations and clinical competencies of staff, and quality assessment performance improvement.

  • Participate in public relation and community activities that promote Signature’s role as an effective member of the hospice and healthcare delivery system.

  • Hire, assign, supervise, and evaluate Registered Nurses.

  • Oversee the functions, activities, and evaluations of all healthcare personnel.

  • Responsible for the orientation and in-service education for all healthcare personnel.

  • Order Review

  • Care Plan review

  • Coordination Note review/gather for Inter-disciplinary team meetings

  • Visit re-assignment approvals

  • Subordinate position workflow monitoring and follow up (Scheduler/Intake/team field clinicians)

  • Supervisory visit assignment and review

  • Must possess a nursing degree from an accredited college or university; BSN degree preferred.

  • Must possess a current, unencumbered, active license to practice as an RN in this state.

  • Experience in hospice/home health setting preferred.

  • Minimum 2 years’ supervisory experience preferred.

  • Current CPR Certification.

  • Must have a valid driver’s license and current automobile insurance.

  • Ability to work within an interdisciplinary team.

  • Excellent observation skills and nursing judgment.

  • Understand hospice philosophy and issues of death and dying.

  • Understand principles of pain and symptom management.

  • Excellent oral and written communication skills.
