  3. Provides patients’ personal hygiene by giving bedpans, urinals, baths, backrubs, shampoos, and shaves; assisting with travel to the bathroom; helping with showers and baths.

  4. Provides adjunct care by administering enemas, douches and non-sterile dressings, preps, ice packs and heat treatments.

  5. Maintains patient stability by checking vital signs and weight; testing urine; recording intake and output information.**Treats the patient’s home environment with respect.

  6. Provides care that maintains patient’s skin integrity to prevent ulcers, skin tears and other damage by changing incontinent patients, turning, repositioning and by applying moisturizers to fragile skin and other areas.

  7. Provides patient comfort by utilizing resources and materials. Reports observations of the patient to Registered Nurse.

  8. Documents actions by completing forms and Electronic Medical Records.

  9. Protects organization’s value by keeping patient information confidential.

  10. Maintains the comfort, privacy, and dignity of patients in the delivery of services to them. Interacts with patients in a manner that displays warmth and promotes a caring environment.

    1. Assists in maintaining a safe, neat clean environment; reports environmental deficiencies such as equipment problems. Ensure that supplies and equipment necessary for patient care are stored in an organized and efficient manner.

    2. Participates in economical utilization of supplies and ensure that equipment is maintained in a clean and safe manner.

    3. Performs all job responsibilities in accordance with prescribed safety and infection control procedures, including through hand washing, use of disposable gloves where indicated and proper disposal of soiled materials.

    4. Maintains professional and technical knowledge by attending educational offerings; reviewing professional publications; establishing personal network.

    5. HIPPA Competency- Maintains patient confidence and protects operations by keeping information confidential.

    6. Adheres to Hospital/Nursing Service policies as evidenced by the Director of Home Health/Hospice.

    7. Works in a variety of environments, including patient homes, Assisted Living Facilities, Nursing Homes, and the Home Health/Hospice office.

    Assembles and maintains equipment required for this position: Field bag contents, personal vehicle, patient assigned care plans and time/mileage sheets. Provides proof of current driver’s license and automobile insurance.

    High School Diploma or equivalent required

    Current Home Health Aide license

    Completion of 20 hour Kansas Home Health Aide class and state exam

    Completion of 12 hours education/training annually as required for Home Health Aides

    BLS required within 30 days of hire

    Experience as a CNA or Home Health Aide preferred
