Work in a leadership role for both identified individuals and the overall household.
- assisting in the assessment, development and implementation of Individual Service Plans
- ensuring that individuals receive appropriate medical care by coordinating doctor and other health related appointments
- assisting individuals in managing their funds
- completing monthly documentation.
Schedule: Tuesday 2p - 10p, Wed 3p - 11p, Thurs 2p - 11p, Fri 2p - 10p & Sat 3p - 10p
Valid driver's license required Required Experience
High school degree and 1-2 years experience working with individuals with developmental disabilities required
Bachelor's degree preferred
