Opened as Mary's Help Hospital in 1912, Seton Medical Center, a 357 licensed bed facility serves the highly diverse 1.5 million residents of San Francisco and San Mateo counties with a comprehensive range of inpatient and outpatient services. Its sister hospital is Seton Coastside, a 116-bed skilled nursing facility located in the coastal town of Moss Beach. Seton Coastside offers the only 24-hour Standby Emergency Department along the 55-mile coastal stretch between Daly City and Santa Cruz.
The Manager of Risk & Patient Safety is responsible to manage specific aspects of Seton Medical Center's patient safety and risk management program with an emphasis on patient safety, and harm reduction. This position will play a critical role in the risk management efforts. In coordination with the Director of Quality & Risk Management, this role will design, coordinate and maintain various aspects of the patient safety and risk management programs for Seton Medical Center.
*MAJOR JOB OBJECTIVES AND RESPONSIBILITIES: *
? Review, investigate and analyze incidents in RM Pro, our online reporting system, for risk and adverse event identification, loss prevention and claims management purposes, including both potential and actual patient injury. Recommend interventions which will enhance the safety and well-being of patients, staff and organization at large.
? Work collaboratively with the Director and other areas/departments involved in risk management (such as human resources, facilities, clinical research, quality and planning, compliance, health information management, IT security, privacy, finance, key clinical areas, etc.) to promote an organization-wide approach to hospital acquired harm reduction as opposed to uncoordinated silos.
? Participate on special projects/committees related to patient safety such as the hospital acquired condition (HAC) reduction, Value-Based Purchasing (VBP) oversight (fall reduction, medication safety, incident documentation, patient safety evaluation system program development, etc. Role will differ with the project ranging from group leader to group participant.
? Manage the patient injuries related to care and "lessons learned" meetings for many patient safety/ risk management issues; create written documentation for appropriate distribution and archival; manage efforts to create policies and procedures for harm prevention purposes.
? Collaborate closely with Director of Quality & Risk to ensure coordination of patient safety/ risk issues.
? Complete VHS Claim Initial Report Form (IRF) on each potentially compensable event and forward to the appropriate parties. Ensure completion of IRF for Notice of Intents and Summons & Complaints. Maintain records and log in issues tracking system and keep Director informed of occurrences.
? As a method of loss prevention and harm reduction, serve as a resource to staff and departments for educational issues specific to patient safety and risk initiatives. Participate in Risk Management educational forums.
? Serve as a Chapter Leader for the Patient Safety Chapters. This includes oversight for the Patient Safety Program, the National Patient Safety Goals and Sentinel Event Alerts. And supervision of the Patient Safety Evaluation System.
? Maintain current knowledge of the regulatory and accreditation standards as they apply to Patient Safety, Error Reduction, Hospital Acquired Conditions, etc.
? Conduct in-depth reviews of high-risk practices through FMEA's, CSA's and other process improvement methodologies including LEAN and Six Sigma. Lead RCAs/intensive reviews as needed.
? Mobilize departmental or administrative support to address unresolved high-risk practices.
? Provide input or author articles for Shared Learning, newsletters, toolkits, manuals and other types of written education.
? Assists in ensuring 24/7 risk management consulting is available to Seton Medical Center for urgent and routine matters.
? Assist Director of Quality & Risk with Patient satisfaction improvement activities.
? Develop annual education plan for own career development including attendance at ASHRM, CSHRM or other events to augment knowledge level; keep educational goals within budget provided.
REQUIREMENTS AND QUALIFICATIONS:
? Bachelor in science of nursing degree
? Current California RN licenses
? Requires 3 - 5 years of background and experience in a medical environment, direct clinical experience preferred.
? Experience in the inpatient, outpatient, and skilled nursing care settings is a plus, as well as prior patient safety/risk management experience.

? Must hold a proven track record in working successfully with groups of people to achieve a desired outcome. An understanding how to manage projects and project teams is essential.
? Ability to communicate both verbally and in writing in a clear and concise manner is imperative. An understanding of how to communicate both positive and negative outcomes is critical to the success of this program.
? Must possess strong problem-solving and interpersonal skills; must be able to work both independently as well as on a collaborative basis within all levels of the organization including physicians, employees and management staff.
? The ability to keep information confidential and to know the appropriate time to share confidential information with the proper individuals is an essential skill for this position.
