SUMMARY: The Home Health Clinical Coordinator Assistant functions under and is directly responsible to the Home Health Clinical Coordinator. He/she has primary responsibility and authority for processing all day-to-day workflow: reviewing and approving MD orders, Plans of Care, completing medication reconciliation, reviewing and processing coordination notes, completing in-home visits as needed. Develops necessary reports as requested by the Clinical Coordinator. He/she should have a working knowledge of reimbursement and regulatory issues and the ability to apply this to daily activities. Successful outcomes of this role include accountability for timely and accurate processing of all daily workflow as identified above, accurate and timely documentation of any in-home visits , , continued and improved outcomes based on generally accepted measures, compliant practice with excellent survey outcomes, positive problem solving, excellent staff training/support/productivity and excellent staff satisfaction. DUTIES AND RESPONSIBILITIES : 1. Responsible for functions, activities and evaluation of all workflow to assure documentation of quality patient care and conformity to agency policies, procedures and professional standards. Evaluates care provided by field staff through a variety of means such as home visits, case conferences, record review, etc. 2.
