Provides oversight and monitors staff activities under supervision of the Program Manager for Home Care/Community Support Programs.
JOB EXPECTATIONS (Essential Functions, Specific Duties and Responsibilities)
Maintain current knowledge of Home Care/Community Support programs, services, policies and procedures to provide oversight of staff in accordance with applicable contract guidelines.
Provide staff orientation and supervision; coordinate staff assignments, monitor staff activities and review required documentation. Conduct employment interviews and make hiring/staffing recommendations for open positions. Prepare and conduct annual performance
appraisals and corrective action discussions as needed with Program Manager.
Assign staff schedules and daily/weekly tasks.
Assist in opening new cases, billing, and filing of required documentation.
Coordinate with funding source, the development, implementation and evaluation of Person Centered Care Plan in coordination with Program Manager and Supports Coordinator.
Complete initial Home Environment Assessment and continue assessments on an on-going basis.
Review documentation included in Person Centered Plans, Care Plan, and Confirmation of Services to assure compliance; document and report inaccuracies or discrepancies to Program Manager.
Maintains “on call” availability as necessary; respond to pager and/or phone calls; follow up with Program Manager to take appropriate action when necessary.
Assist individuals with activities in daily living including but not limited to household tasks, communication skills, social and recreational skills, financial management, and activities in the community.
EDUCATION, TRAINING and LICENSURE/CERTIFICATION
Associate's degree in human services, or equivalent education and experience.
Bachelor's degree preferred.
Three years working with individuals with developmental disabilities, mental illness and/or senior adults in a residential setting.
One year supervisory experience in a related human service field preferred.
KNOWLEDGE, SKILLS, and ABILITIES
Strong knowledge of social service administration/management and staff supervision.
Ability to apply principles of logical thinking to a wide range of intellectual and practical problems.
Advanced ability to read and interpret legal, technical, and financial documents.
Ability to work creatively with staff, physicians, families and superiors.
Ability to display compassion and sensitivity with vulnerable adults who may become angry, hostile, or combative.
Ability to communicate effectively with individuals with varying levels of cognitive and physical impairments.
Ability and willingness to work constructively with individuals from diverse cultural backgrounds.
Ability to maintain a helping role and intervene with client care when appropriate.
Ability to communicate expectations clearly, and to effectively utilize a teamwork approach with staff at all levels.
Ability to add, subtract, multiply and divide all units of measure.
Ability to read, write, speak and understand English language.
Writes legibly with proper use of spelling, grammar and diction. Ability to work independently and with minimal supervision.
Ability to handle multiple tasks simultaneously.
Knowledge of and willingness to utilize basic computer software applications.
Extended hours required.
May be required to safely lift and transfer person served weighing average of 150 pounds on a frequent basis. May be required to push person in wheel chair weighing up to 250 pounds.
