Identifies self to patient by name and profession, explains rationale for service prior to SLP evaluation/treatment
Includes patient in care planning process when reasonable.
Provides for patient safety at all times
Responds to request for service in a timely manner and initiates SLP treatment when appropriate as per established criteria for SLP referral
Contacts physician for clarification when there is a question concerning prescribed patient orders
Communicates inappropriate referrals to physician and obtains D/C order when indicated
Prior to SLP evaluation, reviews patient record and assesses impact of available information on the assessment and treatment process
Conducts a thorough initial assessment of the patient to determine speech/language/oral-motor deficits
Develops and implements an individualized Plan of Care containing achievable/functional goals, timeframes and treatment methods
Develops Plan of Care based on evaluation, reason for referral and team interaction/collaboration
Provides ongoing assessment of patient and revises treatment program as needed based on patient response
Post-Graduate Degree REQ
