The Salvation Army of the Syracuse Area Pathway of Hope Case Manager in Staten Island, New York

Overview

The Case Manager at the Stapleton Corps will work with the families served by the Stapleton Corps to create unique ‘scopes-of-care’ which will lead a family out of poverty while also cultivating a list of strategic partners with resources that can benefit

Responsibilities

  • Cultivate partnerships with other social service providers, churches, and other not-for-profits across Staten Island that offer services that could assist clients being served.

  • Performs casework duties, including meeting with residents, preparing psychosocial evaluations, and obtaining, analyzing, and evaluating data.

  • Develops service plans and makes periodic reports to supervisor on the progress of each case.

  • Carries a caseload and performs record keeping functions as required, case notes, and census statistical reports.

  • Maintains all records with strict confidentiality.

  • Refers residents to agencies and programs as necessary, and establishes a rapport and professional relationship with these agencies.

  • Assists residents in establishing relationships with outside services, and arranges for transportation to appointments.

  • Attends staff meetings as requested by the Corps Officer.

  • Attend training sessions and workshops around the NYC area as requested by Corps Officer.

  • Create process for measuring impact of personalized plans created for clients and measure each case at least two times per year.

  • Prepares workshops and other educations opportunities for clients.

  • Carefully follows the Pathway of Hope model and works with other TSA locations to ensure the greatest possible level of care.

  • Review and determines approval for various forms of support (clothing vouchers, rent assistance etc.) based upon budget provisions.

  • Consistently reports to work on time and is ready to begin work at the start of the shift.

  • Always provides proper notification when late absent.

  • Demonstrates reliability when on-call. Available to work overtime when necessary (only non-exempt employees).

  • Demonstrates the ability to recognize priorities in organization of workflow.

  • Able to perform duties independently, with minimal need of direct supervision.

  • Maintains the confidentiality of clients, shelter/program, agency and/or The Salvation Army.

  • Reports to work well groomed and in compliance with dress code.

  • Demonstrates the ability to interact in a positive and helpful manner with clients, visitors, other staff, volunteers, and co-workers.

  • Reflects commitment to building a supportive work environment and maintains a positive attitude at the workplace and towards job.

  • Projects a good image in dealing with the public and clients.

  • Is willing to make an extra effort to help build a quality case management system.

Qualifications

  • Bachelor’s Degree in Human Services field from an accredited college or university.

  • Working towards a Master’s Degree in a Human Services field preferred

  • Passion for helping individuals escape the cycle of poverty through collaborative work with other community organizations.

  • Willingness to be a part of a The Salvation Army Stapleton Ministry Team

  • Positive attitude and passion for assisting the ministry of The Salvation Army
