Position Responsibilities: 1. Creates an environment and culture that contributes to the company’s mission and vision.

  1. Collaborates effectively with all members of the patient care team within the facility to plan, execute and evaluate the delivery of care to the surgical patient.

  2. Provides a safe, secure, and infection/hazard-free environment.

  3. Supports the healthcare team in the development, implementation, and monitoring of quality initiatives.

  4. Demonstrates high ethical standards in customer service through all interactions with the patients, families, coworkers, staff, other hospital departments, physicians, community and all others.

  5. Creates a high quality, professional clinical environment to facilitate patient care services.

    Department Specific Criteria:

  6. Demonstrates necessary practical, technical and/or specialized knowledge and skills required for the role of physical therapist in accordance with regulatory standards of practice.

  7. Ensures patient dignity and respects patient values.

  8. Operates within ethical standards.

  9. Able to access and reassess the needs of the patient.

  10. Proper documentation of assessments.

  11. Able to communicate the needs of patients.

  12. Consults with other members of the healthcare team including physicians, nurses and ancillary personnel to ensure for the delivery of high quality patient care.

  13. Participates as an active member if the interdisciplinary team.

  14. Provides ongoing education to patient and family as needed.

    1. Establishes and maintains effective working relationships with patients, families, co-workers, physicians, administration, community members, and referral sources.

    2. Communicating results and trends to supervisor.

    3. Provides feedback to supervisor on ongoing basis in regards to concerns, improvements, changes, etc.

    4. Identifies areas needing improvement and utilizes the facility performance process. Actively participates in comprehensive program evaluation system.

    5. Proposes ideas and suggestions that will make processes more efficient and effective for patients.

    6. Serves as an educational resource to patients, families, interdisciplinary team members, physicians, discharge planners, therapists, community members, and other parties as appropriate regarding rehabilitation issues.

    7. Works with supervisor to ensure the program meets all local, state, TJC, regulatory standards.


    o Bachelor’s Degree or higher in Physical Therapy

    o State licensure

    o BLS

    o Direct Physical Therapy experience; one year or more preferred

    o Excellent interpersonal skills/communication skills

    o Excellent organization/time management skills

    o Excellent customer service skills; professional appearance

    o Experience in providing educational programs
