Lifetime Care Nurse Practitioner - Palliative Care Program in Rochester, New York


Summary: Under the supervision of the Medical Director, the Hospice & Palliative Care Nurse Practitioner is responsible to provide hospice and palliative care to patients and their families, including assessment, planning, implementing and evaluating care needs in accordance with accepted standards of practice which supports the philosophy, mission and purpose of Hospice and Palliative Care. This position also provides clinical support to members of Hospice and Palliative Care team and to other staff as requested by the Medical Director.

Essential Responsibilities/Accountabilities:

  • Diagnoses, treats, and prescribes care for assigned hospice and palliative care patients in collaboration with staff physicians.
  • Serves as attending for assigned hospice and palliative care patients.
  • Conducts physical exams, documents clinical findings and signs attestations of face-to face encounters with assigned hospice patients before their 180 day recertification period and for each subsequent 60 day recertification period.
  • Provides documentation of clinical findings as a result of face-to-face encounter to the hospice physician who is considering recertifying the patient for continued eligibility of hospice services.
  • Performs other clinical duties, as requested by the Medical Director.
  • Provides clinical support to all members of the hospice and palliative care team and to the staff of affiliated institutions in developing a plan of care, providing education, training and advice.
  • Participates as an active member of the Interdisciplinary Team (IDT).
  • Counsels and supports patients, families and caregivers regarding their medical condition and plan of care.
  • Participates in on call coverage with the hospice and palliative care physician(s).
  • Advocates for hospice and palliative care services.

  • Regular and reliable attendance is expected and required.
  • Performs other functions as assigned by management.


  • Valid NYS RN license and a Masters in Nursing from an accredited Nurse Practitioner Program are required.
  • Licensed NYS Nurse Practitioner (NP) in a specialty area that permits them to diagnose, treat and prescribe adult patients ages 18 and older.
  • NP certification from AANP, ANCC National Board for Hospice and Palliative Nurses preferred or agreement to seek certification within 12 months of employment.
  • Unrestricted Federal DEA Registration and DEA number.
  • Must demonstrate solid interpersonal, organizational and time management skills.
  • Basic computer skills are necessary, including but not limited to the ability to navigate on a personal computer within a Windows based operating environment; the ability to access and create basic e-mail messages; and the ability to open, edit, and save basic electronic documents if necessary.
  • With training provided, must demonstrate the ability to learn and utilize new systems, computer applications and operating environments as necessary.
