Lifetime Care Heatlh Steps Specialist (Registered Nurse) in Rochester, New York
Description The Healthy Steps Specialist is responsible for the delivery of Healthy Step program components to assigned Healthy Steps babies and their families. The duties and responsibilities for the care to be delivered by the HSS are identified and defined by the Healthy Steps Program Coordinator. Healthy Steps visits may occur in the physician office or the patient’s home.
Essential Responsibilities/Accountabilities: 1. Conducts office visits, either jointly with the physician/PNP or linked with physician/PNP during scheduled office hours or by appointment. 2. Conducts periodic home visits to support and enhance mothers and fathers in overseeing the growth and development of their child. Initial visits will focus on infant behavior and nutrition with an emphasis on sharing information that enhances parents’ understanding of the infant’s behavioral clues. Subsequent visits will focus more on such behavioral and developmental issues as attachment, limit-setting, discipline, play and temperament. 3. Assesses developmental progress at office and home visits and promotes parental knowledge of their developing child, helping them to appreciate areas of strength and support growth in all areas of development. 4. Develops and maintains a resource and referral file to assist families in finding timely and appropriate community-based programs and resources. 5. Provides referrals to appropriate community resources as appropriate; follows up as needed. 6. Helps parents understand how to manage common behavioral challenges like sleep, colic, feeding and toilet training. 7. Facilitates parent groups and/or collaborates with organizers of existing parent groups in the community to provide programs for Healthy Steps families 8. Operates a developmental and behavioral telephone information line to provide appropriate information, timely interventions, and family support. 9. Encourages use of Reach Out and Read activities. 10. Creates and monitors a parent-to-parent bulletin board to facilitate the development of an informal resource network. 11. Facilitates initial training and ongoing informational/training support to office staff on Healthy Steps procedures. 12. Reinforces interventions as assigned by the Healthy Steps Program Coordinator 13. Systematically measures and documents patient response to teaching and interventions performed. 14. Assists in the implementation of patient/family teaching plans to promote well baby objectives. 15. Conferences with the Healthy Steps Program Coordinator to discuss babies and parents participating in the Healthy Steps program 16. Documents all Healthy Steps and coordinating activities in a timely fashion according to agency policy. 17. Practices according to Agency and community standards. 18. Attends required staff meetings, inservices, and/or supervising conferences when requested. 19. Performs other job-related duties as assigned. 20. Requirements Graduate of an accredited school with a degree in nursing, social work, nutrition, physical therapy, speech therapy or other appropriate educational background. A valid NYS license in specified field. Minimum of 4 years experience providing developmental pediatric interventions. Position requires reliable transportation for independent travel between office, homes and physicians office visits on a daily basis. Must demonstrate strong interpersonal skills.
