As a Speech Language Therapist, you will ensure the therapy care plan is being followed. Working alongside Rehab Aides, you will work in collaboration with physicians and therapists to ensure the plan of care for the patient population.

  • Initial and on-going systematic patient assessment

  • Timely and accurate documentation

  • Promoting continuous quality improvement

  • Teaching and counseling patients/families

  • Setting goals and developing treatment plans

  • Incorporating patient’s and family’s goals into treatment plans

  • Works cooperatively as an ancillary interdisciplinary team member to identify and solve patient-specific and facility-wide needs and improve operations

  • Implement company standards of care, policies, and procedures

Minimum Education & Experience (Including Licenses)

  • Current state licensure or eligibility for state licensure as a Speech Language Pathologist required.

  • Current Certificate of Clinical Competence (CCC) in Speech Language Pathology from the American Speech-Language & Hearing Association or eligibility for Certification pending professional experience required.

  • Certified BLS or completion in first 90 days of employment required.

  • Home Care Requirements: One or more years of clinical experience and valid state Driver’s License.
