Position Summary

Evaluates all assigned patients and develops, implements and modifies treatment plan in accordance with the patient’s need and physician direction. Responsible for delegation and direction of all discipline specific therapy services for those assigned patients. Ensures design of functional treatment programs allowing for effective return to community activities. Serves as an educator of patients, families, students, coworkers and self. Performs other professional duties associated with the care of the patient, development of discipline specific therapy services and design of program delivery.

  • Accomplishes individual and team goals and objectives established by supervisor based on prior performance evaluation.

Specific Duties

  • Evaluates assigned patients and establishes appropriate treatment goals and treatment programs.

  • Provides ongoing evaluation of patient’s status and goals, modifying treatment program for timely progression.

  • Develops comprehensive goals for patient treatment to maximize patient outcomes and return to community activities.

  • Coordinates treatment program in conjunction with other clinical/therapeutic services, as well as available community resources. Attends appropriate clinical meetings to facilitate coordination.

  • Provides for patient education to include development of comprehensive and progressive home programs with instruction to patient, family and other caregivers as appropriate to facilitate a safe and effective discharge.

  • Prepares for patient discharge inclusive of ordering of necessary equipment and recommendations for therapeutic services ensuring the continuum of care.

  • Evaluates patient in actual or simulated home, school, work or community environments as necessary to facilitate return to community activities, making recommendations for necessary adaptations or training.

  • Identifies and manages various levels of care necessary to insure care plan delivery. Delegates and supervises the utilization of supportive personnel to maximize patient outcomes and insure effective utilization of services.

  • Carries full patient caseload, ensuring management of patient schedule. Notifies supervisor of scheduled openings and concerns with caseload management.

  • Completes medical record documentation in a clear, concise, accurate and timely manner and ensures accurate and timely charge entry using approved formats in compliance with state, federal and intermediary guidelines and in accordance with established policies and procedures. Ensures care is provided under valid physician orders and within insurance approval, as indicated and necessary for clinical care of the patient.

  • Demonstrates flexibility, team building and active participation in departmental functions in order to promote an efficient and positive work environment.

  • Participates in student program as assigned and appropriate, providing a positive learning environment for students with appropriate supervision, development of learning objectives and completion of student evaluations. Will attend all student lectures, and begin developing public speaking skills required to facilitate student lectures.

  • Identifies and initiates own professional growth, actively pursuing professional development through continuing education, self study and involvement in professional activities and organizations. Participates in a minimum of one continuing education offering relevant to the strategic objectives of the organization and the professional development plan discussed with the manager.

  • Maintains communication with supervisor related to service needs such as necessary supplies, equipment and clinical training in order to effectively complete assigned duties.

  • Actively participates in patient and professional advocacy efforts that may include lobbying activities, health fairs, community level events, professional memberships, fundraising/charitable activities.

  • Adheres to discipline specific State Practice Act and other regulatory agencies. Maintains professional licensure in good standing and completes all state mandated continuing education requirements. Performs in an ethical manner in accordance with discipline specific professional code of conduct.

  • Performs other duties as requested.

Minimum Education & Experience (Including Licenses)

  • Current state licensure or eligibility for state licensure as a Speech Language Pathologist required.

  • Current Certificate of Clinical Competence (CCC) in Speech Language Pathology from the American Speech-Language & Hearing Association or eligibility for Certification pending professional experience required.

  • Certified BLS or completion in first 90 days of employment required.

  • Home Care Requirements: One or more years of clinical experience and valid state Driver’s License.
