Under the supervision of the Director of Education and/or Manager of Education, the Coordinator and /or Diabetes Educator is responsible and accountable for individual practice and when applicable appropriately delegates and supervises tasks to provide optimum patient care. Participates in establishing, maintaining, and improving patient’s health care goals through patient assessment, planning and implementation of interventions and evaluation of patient outcomes. Maintains a state-of-the-art approach to diabetes care and self-management training. Provides individual and group education to diabetes patients that are accurate and based on current American Diabetes Association (ADA) standards. Assists with coordinating diabetes patient care throughout the continuum, collaborating with all disciplines to facilitate optimum patient outcomes. Serves as a clinical resource and consultant for issues related to diabetes for patients, physicians, staff and the community.Applies principals of adult learning when providing instruction. Uses appropriate teaching methods and materials to meet the age-specific needs of patients/families across the life span. Communicate effectively with other care providers (physicians, nurses, business office, and medical records) to provide for continuity of patient care and to meet the needs of the customers we serve.
JOB REQUIREMENTS: Education: Graduate from an accredited school or university REQUIRED: Bachelor’s Degree in Nutrition and Food Science or Registered Nurse PREFERRED: Master’s Degree
Licensure/Certification REQUIRED: Current RN license in Texas or Licensed Dietitian, BLS certification PREFERRED: Certified Diabetes Educator Experience required or preferred: PREFERRED: One year clinical experience preferred
