  • Select, train, and manage designated staff, fostering a work environment that encourages teamwork, promotes personal growth and learning, couples empowerment with accountability, learns from failure and celebrates success.

  • Manage all Service Coordination activities on their designated island ensuring all health functional assessments, service plans, and self-direction activities are completed according to Med-QUEST timeline requirements.

  • Administer an effective Service Coordination program through multi-disciplinary teams with the goal of improving health care outcomes and member satisfaction.

  • Identify opportunities and develop programs to improve the operational efficiency and cost-effectiveness of Service Coordination processes and programs.

  • Oversee the performance of Community Case Management Agencies on their assigned island and manage vendor contracts to maintain a positive working relationship and ensure that deliverables are being met in accordance with contract requirements.

  • Establish and maintain cooperative and productive professional relationships with service coordinators and case managers at various facilities and community organizations.

  • Act as a liaison and mediator between participants, families, and service providers addressing any concerns that may arise.

  • Other duties as assigned.

Minimum Qualifications:

  • RN or LSW with a current Hawaii license.

  • Three years experience in delivering services to the LTSS populations and 2 years experience in managing programs and people, budgeting, planning, and project management or equivalent combination of education and work experience.

  • Working knowledge of the operation of managed care programs.

  • Good verbal and written communication skills.

  • Effectively work independently, managing multiple priorities simultaneously and under strict time constraints.

  • Working knowledge of PC word processing, database and spreadsheet programs.

  • Requires safely operating an insured automobile for travel to off-site locations to conduct and accomplish business related activities.

  • Must have valid driver’s license, access to an automobile with current license, registration and no fault insurance.

  • Occasional travel to the Neighbor Islands or the mainland.
