Company:Hawaii Medical Service Assn.Job Title:Manager, Service Coordinator - Dual Care (Oahu)Exempt or Non-Exempt:ExemptIsland:OahuCity:HonoluluLocation:KaimukiEmployment Type:Full TimeJob Summary:Manage and oversee the performance of Service Coordinators on their assigned island/territory and direct the daily Service Coordination operations related to Special Health Care Needs (SHCN) in compliance with HMSA policies, contractual standards and Federal and State law.Priority Parking - Driving is required (50% or more of the essential duties in the job description requires traveling to off-site meetings with various resource agencies and to supervise staff.Duties and Responsibilities:
Select, train, and manage designated staff, fostering a work environment that encourages teamwork, promotes personal growth and learning, couples empowerment with accountability, learns from failure and celebrates success.
Manage all Service Coordination activities on their designated island ensuring all health functional assessments, service plans, and self-direction activities are completed according to Med-QUEST timeline requirements.
Administer an effective Service Coordination program through multi-disciplinary teams with the goal of improving health care outcomes and member satisfaction.
Identify opportunities and develop programs to improve the operational efficiency and cost-effectiveness of Service Coordination processes and programs.
Oversee the performance of Community Case Management Agencies on their assigned island and manage vendor contracts to maintain a positive working relationship and ensure that deliverables are being met in accordance with contract requirements.
Establish and maintain cooperative and productive professional relationships with service coordinators and case managers at various facilities and community organizations.
Act as a liaison and mediator between participants, families, and service providers addressing any concerns that may arise.

RN or LSW with a current Hawaii license.
Three years experience in delivering services to the LTSS populations and 2 years experience in managing programs and people, budgeting, planning, and project management or equivalent combination of education and work experience.
Working knowledge of the operation of managed care programs.
Good verbal and written communication skills.
Effectively work independently, managing multiple priorities simultaneously and under strict time constraints.
Working knowledge of PC word processing, database and spreadsheet programs.
Requires safely operating an insured automobile for travel to off-site locations to conduct and accomplish business related activities.
Must have valid driver’s license, access to an automobile with current license, registration and no fault insurance.
Occasional travel to the Neighbor Islands or the mainland.
