· Clinical Pharmacist Specialist in Ambulatory Care Parkview Pharmacy is accepting applications for a clinical pharmacy specialist in Ambulatory Care. Responsibilities: The clinical specialist will provide direct patient care as part of the primary care medical treatment team in an ambulatory setting. The Parkview Physician Group team includes physicians, nursing, medical assistants, and support staff. The pharmacist will work collaboratively with to provide pharmaceutical care to ensure safe, appropriate, and cost effective therapies. This includes, medication therapy management, reconciliation, education, facilitation of pharmacy benefit utilization to optimize adherence, and optimizing transitions of care. This position will facilitate the expansion and enhancement of clinical pharmacy services across the ambulatory environment. Developing and leading quality initiatives that will make a difference across a variety of ambulatory settings is an example of the professional reward that this clinical specialist can expect. This position will have the support of other clinical pharmacists and a clinical informatics specialist to facilitate opportunities. Parkview Health has provided specialty pharmacy services in a variety of areas for over 20 years. Pharmacy is already engaged in ambulatory services in Falls Prevention, Hypertension, Hyperlipidemia, Pediatrics, Anticoagulation, and growing. Inpatient pharmacy services are extensive and include provision of anticoagulation dosing and monitoring, pharmacokinetics, pain management, antimicrobial stewardship, patient counseling and education, and nutritional support services. PRMC, a member of Parkview Health, is a major regional medical center located in Fort Wayne, Indiana. Qualifications: Candidates must possess a pharmacy degree from an ACPE-accredited institution and have completed a PGY1 pharmacy residency. A PGY2 residency in Oncology/Hematology (or equivalent experience) is highly preferred. Pharmacy licensure, or eligibility, in Indiana is required. Candidates must have a high level of clinical practice skills, enthusiasm for teaching, and the interpersonal skills needed to interact effectively with students, health professionals, and patients.
