Nurse Navigator Oncology, Full Time Colorado Blood Cancer Institute (CBCI) Out Patient Clinic Position Summary:This position supports the mission of Presbyterian/St Luke’s Medical Center of optimizing quality of life of all those served by expanding beyond the traditional focus on medical needs to the needs of the whole person. Within scope, the nurse navigator oversees and directs the care of the BMT or Hematology-Oncology patient and is responsible for collaboration with all disciplines to provide well-coordinated patient care. This position functions to navigate blood cancer patients through the continuum of care including diagnosis, treatment, follow-up and long-term care needs. This includes coordination of chemotherapy orders, prescriptions and individualized follow-up. The nurse navigator demonstrates competence in the ability to triage patients over the phone and in person. The nurse navigator demonstrates the ability to interpret laboratory and radiologic test results and communicates results to physician as necessary. The nurse navigator provides patient and family education relative to the patient’s diagnosis and treatment process. The nurse navigator demonstrates collaboration, professionalism, and effective communication in all interactions with physicians, the administrative team, PSLMC employees, patients, caregivers and referral sources. The nurse navigator acts as an ambassador for the bone marrow transplant program when communicating with other healthcare professionals, payors, and the community. When the need arises, will perform other duties as assigned by Lead.

A. Licensure/Certification/Registration: Current licensure in the State of Colorado as a Registered Nurse; BLS – Healthcare provider. OCN certification within 18 months of hire. B. Education: Graduate of an accredited school of nursing, BSN preferred.
C. Experience: Three to five years experience in hematology-oncology or bone marrow transplant care preferred.
D. Special Qualifications: Ability to communicate effectively with patients, families, physicians, and other health care team members. Ability to manage a team and work with others within a team to ensure quality patient care. Strong critical thinking skills. Desire to provide compassionate, holistic care. Able to work independently without close supervision.
Degree of supervision required: Involves general guidance and direction by the CBCI Clinic Manager. Employee will be expected to perform most job duties independently and in accordance with established departmental and hospital policies and procedures.
Ages of Patients Served: This position requires knowledge of growth and developmental needs of patients and families. Personnel in this position must recognize and respond to the behaviors of patients and families in each age category. The employee must be able to demonstrate the special skills required to care for patients and their families according to standards. (The skills and knowledge needed to provide such care may be gained through education, training or experience.)
