Scope of Position
Case Managers provide comprehensive case management and counseling services to adults with a history of mental illness, in an outpatient and field-based setting to Spanish speaking clients. The ideal candidate will function with, and maintain a "whatever it takes attitude”.
Conducts/facilitates individual and group counseling and provides routine case management including assessment, intake, education, crisis intervention, and exit interviews for adults or adolescents with a history of mental illness and/or substance abuse.
Interfaces with such agencies as the Department of Mental Health, Criminal Justice Services, Public Health and other related community agencies and resources to give and receive information relative to clients.
Maintains documentation of all counseling sessions, substance abuse services /all other services, and ensures compliance with agency policies/procedures, contracts, and other regulatory agencies.
Supervises program participants with medication compliance, and drug screening as needed.
Provides crisis intervention, and facilitates conflict resolution among the program participants.
Prepares education materials for staff training and presentations.
Chaperons group sponsored trips and social functions; may drive seven-passenger van to transport program participants, escorts/transports individuals to appointments outside facility.
Develops and maintains counselor skills through seminars, workshops, or other forms of training/education.
Actively participates in all team and staff meetings, community meetings and supervision/consultation.
Bachelors Degree in related field preferred.
1-2 years experience working with individuals with a history of mental illness in a social service/mental health environment.
Must be able to provide case mangement services in both English and Spanish (Read, Write, and Speak)
Knowledge of case management, crisis intervention techniques, community resources, and the ability to match clients with a history of mental illness to community support resources.
Proven comprehension of the recovery model desired.
The ability to provide services in a language other than English preferred.
Knowledge of program HIPAA, DMH, policies and documentation guidelines, licensing, and other state/federal regulations relating to service programs for adults with a history of mental illness.
Demonstrated advocacy skills.
Strong written and oral communication and PC skills.
Current California drivers license and a driving record acceptable to the Agency's insurance carrier.
