UNC Health Care TRANSPLANT NURSE COORDINATOR - HEART PRE TRANSPLANT in Chapel Hill, North Carolina
The Heart Transplant Coordinator provides support and education to patients throughout the transplant process. He/she works closely with the multidisciplinary team to oversee and/or coordinate the clinical, psychosocial and financial needs of the transplant patient, coordinate tests and consults to determine transplant candidacy and maintains the program's waiting list which includes providing regular follow up on patient status while waiting for transplant. He/she also monitors inpatient status, discharge needs and follow-up care per program standards. The Heart Transplant Coordinator documents and maintains patient transplant information in the electronic medical record to ensure regulatory compliance and participates in quality activities. Spanish speaking highly preferred.
Requires graduation from a state-accredited school of nursing, BSN and four years of professional nursing experience to include two years in transplant, critical care, or disease specific area. Must be licensed to practice as a registered nurse in the state of North Carolina by the North Carolina Board of Nursing.
