Description

RN Coordinator of patient care. Coordinates evaluation process for patients to ensure effective use of hospital and clinical resources. Facilitates the scheduling of studies or procedures for diagnostic purposes utilizing fiscal responsibility for patient and hospital costs. Assists nurses and other staff to assess, plan, implement and evaluate the care of the patient. Communicates with insurer case manager on an as needed basis.

Education. Educates patients along the continuum to assist understanding of processes and procedures surrounding the course of treatment for the specific patient population. Refers patients or families for assistance utilizing health systems and community resources as appropriate. Develops patient education materials or identifies external resources that are appropriate for patient education. Educates nurses and other multidisciplinary team members about patient care issues to promote positive outcomes for the patients.

Participates in the development and implementation of protocol and standards of care for the patient in collaboration with the multidisciplinary team.

Patient Assessment- Anticipates problems before they occur and implements measures to minimize risk to the patient. Assesses patient using a holistic approach, including consideration of biophysical, psychosocial, family support system, self-care, educational as well as the patient's values and beliefs. Assess subtle changes in patient status. Clearly and concisely documents initial and ongoing patient assessment.

Quality Improvement. Coordinates and implements collection, analysis and interpretation of data related to multiple protocols for the specific population of patients. Maintains and implements methods to track research data for entry in database as designated. Monitors the process and outcomes of routine care to determine quality improvement need.

Reviews and monitors clinical trials and outcomes for all patients. Serves as a community consultation resource through media, telephone, and internet links for information about course of treatment in the specific patient population.

40hr/week 08:00am - 5:00pm

Qualifications

Graduation from a state-accredited school of professional nursing. Three (3) years of current nursing experience in the specific area of the patient population. Licensed to practice as a Registered Nurse in the state of North Carolina.
