Position Summary

The Clinical Mentor functions as a clinical expert, role model, and resource. Provides leadership for clinical initiatives, performance improvement, and other activities to improve patient care. Has knowledge of community health concepts, principles of ambulatory clinical nursing care, case management and care coordination. Assumes responsibility and accountability for facilitating, communicating and collaborating with the healthcare team and the patient/ family, utilizes best practice to maximize patient engagement and patient self-management efforts. Promotes the optimal health, well-being and safety of the patient through use of the nursing process and in accordance with patient care standards, guidelines and the State Nurse Practice Act. Demonstrate personal accountability for relationship-based care, organizational mission and core values.

  • Minimum Education Requirements*

Graduate of an accredited School of Nursing BSN Preferred

  • Minimum Experience Requirements*

Three year experience required Nursing skills and knowledge base specific to specialty

  • License/Certifications*

Current Colorado RN license/ Current RN license from a participating state in National Licensure Compact Current BLS from American Heart Association required, ACLS or other certification may be required depending on patient population RNs Certification in Ambulatory Nursing through the AAACN, or strongly preferred upon hire or agree to obtain within 1-year of eligibility. Other applicable nursing certifications may be considered

☑ Medium Work - exert/lift up to 50 lbs. force occasionally, and/or up to 20 lbs. frequently, and/or up to 10 lbs. constantly
