Please note that this Field Case Manager position requires travel within a 50 mile radius of current residence. Current local areas of business need focus include the greater Columbus, Cincinnati, or Dayton, OH areas. Candidates within the greater Cleveland, OH area may be considered depending on future business need. In order to be considered for this position, you must hold an active Registered Nurse (RN) license.
The Registered Nurse (RN) Field Case Manager utilizes a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individuals benefit plan and/or health needs through communication and available resources to promote optimal, cost-effective outcomes. This will be accomplished through telephonic outreach along with face to face visits if the member meets criteria and lives within a 50 mile radius from the case managers home/office.
Fundamental Components of the Field Case Manager include, but are not limited to:
Utilize clinical skills in a collaborative process to assess, plan, implement, coordinate, monitor and evaluate options and services to facilitate appropriate healthcare outcomes for members.
Collaborate to assess, plan, implement, coordinate, monitor and evaluate options and services to meet an individuals health needs through communication and available resources to promote quality, cost-effective outcomes.
Active Registered Nurse (RN) licensure required.
Minimum of 3+ years case management experience strongly preferred.
Minimum of 5+ years of total clinical practice experience as a Registered Nurse strongly preferred.
Ability to travel within a designated geographic area for in-person case management activities
Bilingual proficiency a plus!
Knowledge of laws and regulations governing delivery of rehabilitation services preferred.
The minimum level of education desired for candidates in this position is a Associate's degree or equivalent experience.
LICENSES AND CERTIFICATIONS
Certified Case Manager (CCM) is preferred, but not required.

Technical - Desktop Tools/Microsoft Word/1-3 Years/End User
This role is a field-based travel position within the state of Ohio.
