General summary/purpose: The purpose of this position is to work in collaboration with cystic fibrosis (CF) physicians, social workers, and other team members to deliver and coordinate mental health care services for patients of the Johns Hopkins Adult Cystic Fibrosis Care Center.

The individual filling this position will be fully dedicated to leading, organizing, and administering mental health coordination services for the approximately 333 patients in the Johns Hopkins Adult Cystic Fibrosis Program. This individual will work closely with the current CF Social Worker, CF physicians and nurses to develop support programs for adult CF patients.

Services will include performing depression and anxiety screening of CF patients during clinic visits. The mental health coordinator will collect and interpret diagnostic data, and will follow the results of the depression and anxiety screening results longitudinally. He or she will assist in maintaining and building a database of mental health professionals to use for CF patient referrals. Additionally, he or she will work closely with the care team on personalized case management, providing motivational and adherence counseling and support. The Johns Hopkins Adult CF Program is committed to using all of its existing resources to aid in training and making the individual filling this position successful.

Specific duties & responsibilities:

  • Perform quarterly anxiety and depression screening of existing patients in the Johns Hopkins Adult Cystic Fibrosis Program

  • Coordinate mental health care interventions and referrals, including counseling and/or medication management for patients scoring in the moderate to severe range on screening

  • Develop and document a personalized mental health plan for the patients in collaboration with other CF team members

  • Expand an existing referral database of mental health professionals in Maryland, Northern Virginia, Delaware and Southern Pennsylvania. Assist with developing a network of community-based mental health providers for patients needing to receive mental health care locally

  • Work closely with team to provide ongoing case management of Adult CF Patients

  • Consult with and refer patients with complex psychiatric needs to psychiatric physicians as needed

  • Communicate with primary CF physician about mental health services provided and collaborate with physicians and team members as needed as part of a cohesive team-care model

  • Provide educational tools for community-based providers about CF and serve as a resource for these providers

  • Assist with data collection and quality improvement efforts related to CF mental health

  • Provide mental health counseling to CF patients

  • Other duties as assigned

Minimum qualifications (mandatory): Master’s degree in social work, psychology, or related field required. Licensure as LCSW or other license required per related discipline.

Preferred qualifications:

Special knowledge, skills, and abilities: Successful completion of required field placement work for graduate degree required. Excellent communication and organizational skills are required.
