Job Summary

The Case Manager, RN is responsible for performing a comprehensive MDS assessment of all residents and coordinating the RAI and plan of care (POC) process. The Case Manager, RN is responsible for developing each resident’s comprehensive plan of care and ensures that it addresses effectively the current needs of the resident in collaboration with the Unit Manager, RN. The Case Manager, RN also works with the unit manager, unit staff and IDT (inter-disciplinary team) to guide and oversee the implementation of the plan of care. The Case Manager, RN performs focused assessments on all residents to ensure that residents can maintain an optimal level of functioning.

Qualifications - Minimum

Qualifications – At least 1 year of current MDS experience. The candidate must have worked in an MDS role within the past 2 yrs. The candidate must have a working knowledge of OBRA and PPS regulations and scheduling, as well as CAA completion. Must be detail oriented with ability to work within timelines for assessment completion. The candidate must have basic knowledge of RUG system and case mix optimization. ICD-10 coding experience preferred.

Works with Unit Manager, RN to interpret symptoms and implementation of the POC.

Formulates diagnoses sheet from documented diagnosis in the medical record and monitors updated signatures by physician.

Conducts POC meetings with interdisciplinary team, resident and/or family member(s)/responsible party. Reviews and evaluates effectiveness of interventions and goals of the Plan of Care.

Participates in the survey process as assigned.

Works closely with Licensed Social Worker to establish POC meeting schedule.

Participates in nursing sub-committees as assigned.

Attends monthly Nurses Meetings.

Performs performance improvement audits as assigned.

Reviews quality indicators and takes remedial action where warranted.

Reviews and verifies accurate Resident Care Card upon completion of MDS, quarterly and in the event of a significant change in status.

Interacts with family members, helping them to understand disease process and treatment plan/care plans. Solicits feedback from resident and family to gauge effectiveness of coaching plan and modifies as needed.

Reviews all falls with Unit Manager, RN making appropriate change in intervention to the POC.

Uses computer to enter MDS data appropriately.

Reviews chemical and physical restraint use.

Interacts with and educates resident, families and staff regarding MDS Process and Plan of Care. Challenges and/or corrects inaccuracies as necessary to ensure resident safety and wellbeing and communicates such to Manager, Case Management and Manager, Nursing Services.

Acts as reference to Unit Managers.

Works collaboratively with other members of the Inter-disciplinary team to develop discharge plan.
