The On Demand Replacement Home Alliance Coordinator (RHAC) provides services to Hope support recipients through their leadership of an Assisted Living Home. The On Demand RHAC replaces the HAC when that person is on leave for an extended period or while a new HAC is being hired. The On Demand RHAC is responsible for overall management of the home. This position ensures the health and safety of the support recipients and provides a wide range of services. The On Demand RHAC position includes teaching of living skills, social and communication skills, and other needed life skills for social independence and community inclusion. This position facilitates communication between all members of the support recipient’s team. The On Demand RHAC leads the team of Individual Support Specialist (ISS) , Behavior Health Associate (BHA) and relief staff working in the home, ensuring the implementation of the Plan of Care/Support Plan.
This position requires that the On Demand RHAC live in the home of the individuals receiving supports. The On Demand RHAC is responsible for being in the home unless accompanying support recipient in the community, conducting agency business, on approved time away, or on a designated day off.
The On Demand RHAC may also work in homes replacing an ISS as needs arise. This position may be scheduled for between 10 and 16 hours per day to cover ISS shifts. The On Demand RHAC needs to be very flexible to move from home to home on short notice, when assigned by the supervisor based on need.
Provides overall leadership and management of the Home.
Supervises, develops, evaluates, counsels, and disciplines Individual Support Specialists/Behavior Health Associates working in the home through teaching, coaching and mentoring, as directed by the Community Support Supervisor. Shares information from Agency and department meetings with staff as applicable. Shares all pertinent information with the relief staff before taking time off, and with the returning or new HAC before leaving.
Operates the home within Agency budgetary guidelines and each support recipient’s. Responsible for the home and gas credit cards, cash, receipts, and monthly reports. Responsible for the support recipients’ cash, receipts, and monthly reports. Responsible for staff allocations in alignment with budget.
Ensures wholesome meal preparation, focusing on individual dietary needs and healthy personal and cultural preferences. Ensures appropriate cleanliness, maintenance, and safety of physical site (home and grounds) and home vehicle.
Assists in the development of interaction guidelines and Plans of Care/Support Plans. Facilitates, coordinates, and/or participates in home, behavioral health, and other team meetings as required.
Provides, or schedules staff to provide, personal care and support services required for each support recipient, including, but not limited to, activities of daily living and community activities. Ensures each individual participates safely in a wide range of age-appropriate activities. Ensures goals and objectives are being worked on as outlined in the Plan of Care/Support Plan.
Coordinates, and provides if necessary, transportation to meet support recipients’ schedules, including public transportation and rides to work, appointments, and activities.
Monitors support recipients’ health. Ensures the support recipients follow medical orders and correctly take medication (and documents the same), as specified by their medical practitioners. Schedules medical appointments and medication refills. Accompanies and participates in medical appointments as necessary. Accesses emergency services as needed.
Develops staff schedules to meet the needs of the support recipients, ensures completion of shift tasks, schedules staff meetings as needed, and provides training as assigned. Schedules vacation coverage. Plans for staff safety.
Maintains consistent positive communication, including with the family and/or the guardian of the support recipients, Agency team members, care coordinators, medical professionals, and state licensing personnel.
Verifies, approves, and quality assures time sheets, daily log notes, service notes, goal tracking, and other documentation generated by staff working in the home.
Completes and routes all required Agency paperwork and documentation in a timely and accurate manner, including the timely submission of all maintenance requests, vehicle safety checklists and accident reports, monthly home reports, and incident reports.
Maintains Home License and requirements for re-licensure. Collaborates with the Community Support Supervisor and Adult Assisted Living Licensing Manager.
Education and Experience:
- Required: Bachelor’s degree in gerontology, health administration, or another health related field
Completion of an approved management or administrator training course and at least one (1) year of documented experience relevant to the population of residents to be served
Documented completion of a certified nurse aid training program approved by the Board of Nursing and have at least one (1) year of documented experience relevant to the population of residents to be served
Two (2) years of documented experience relevant to the population of residents to be served
Any combination of the above sufficient to fulfill the duties of an Administrator.
- Strongly Preferred Experience (may be combined with above experience):
Two (2) years of experience with people who experience developmental disabilities.
One (1) year supervisory or administrative experience. (May be obtained from a range of experiences.)
